Application forms for the class beginning summer 2010 are available below. Applications are accepted from July 1 to December 31, 2009.

The following steps summarize the application requirements. All forms required for application are available for download in PDF format below. Applications require signatures and payment by check, and cannot be submitted online at this time.

STEP 1:
Contact agencies required to send information directly to the KU Department of Occupational Therapy Education.

  1. Transcripts
    Arrange to have one official copy of transcripts from each college and university that you have attended sent directly from their records office to the KU Department of Occupational Therapy Education. Copies are not acceptable. International transcripts require additional analysis and may require extra time to complete.

  2. Volunteer/Work Verification
    Have a supervisor document your Work or Volunteer experiences up to and including the past five (5) years. Forty (40) hours must be documented and at least 50% of the time must be direct contact with individuals with special needs. Note that the completed form is confidential, and should be sent directly to our admissions committee by that supervisor. Make additional copies of the Confidential Volunteer/Work Verification Form as needed. In order to expedite the timely return of these forms to our office, you should provide a stamped envelope with each verification form you disperse.

  3. Recommendation Letters
    Request a letter of recommendation from three (3) people. Letters may be from a supervisor, professor, pastor, coach or other adult who can attest to the applicant's potential for sucess in the program and in the profession.

  4. International Students
    If English is a second language, applicants must take the Test of English as a Foreign Language (TOEFL), and arrange for scores to be sent directly to the KU Department of Occupational Therapy Education. A satisfactory score on the TOEFL must be obtained within the previous two (2) years (see guidelines for TOEFL).

STEP 2:
Complete all required application forms and documentation and mail in one packet to the Department of Occupational Therapy Education.
Use the appropriate application packet below. The Prerequisite Review Form should be completed by typing directly into the fields on-screen. This will avoid delays caused by hand-written entries. Adobe Acrobat Reader is required to view and print the forms.

Additional Information for Applicants
After submitting application materials, students must meet a number of additional requirements and submit additional documentation prior to entering the program. Please review this information now so you'll be prepared. Once accepted, a position in the next class must be reserved by signing a contract of acceptance and providing a $400 non-refundable deposit. The deposit amount will be applied toward program fees upon officially entering the program.

In addition, all accepted individuals must provide proof of current certification in First Aid/CPR, current health insurance coverage and pass a criminal background check.
