STEP 1: Documents not provided directly by the applicant.
Contact the following agencies to request information be sent directly to the KU Department of Dietetics and Nutrition:

  1. Arrange to take the Graduate Record Examination (GRE) General Test. Your application cannot be completely processed until we have received your scores. Use institution code 6895 and department code 0214 to have an official copy of your scores mailed directly to our office. The GRE must have been taken within the past four (4) years.

  2. Arrange to have two official copies of transcripts from each college and university that you have attended sent directly to the department.

  3. A verification statement or declaration of intent form is required. This form is completed by your didactic program director and must have an original signature.

  4. If English is a second language, applicants must take the Test of English as a Foreign Language (TOEFL), and arrange for scores to be sent directly to the KU Department of Dietetics and Nutrition. A satisfactory score on the TOEFL must be obtained within the previous two (2) years (see guidelines for TOEFL).

STEP 2: Documents provided by the applicant by mail.
Use the appropriate application packet below. We suggest you complete the forms by typing into the fields using your computer before printing them out . This will avoid delays caused by hand-written applications. Adobe Acrobat Reader is required to view and print the forms. Please complete all forms completely.

This is also a good time to contact our office to speak with an academic adviser who will help guide you through the admission process.

STEP 3:
Applicants must complete the computer match process and indicate KU as one of the desired institutions for enrollment by February 15. For information on the process and computer match forms, contact your didactic program director or D&D Digital Systems at www.dnddigital.com.
Inquiries or Updates: Please call (913) 588-5355 for inquiries about the application process or to check the status of a submitted application.

Notification: In April, D&D Digital notifies all internship applicants of their match status. Matched applicants must contact the program director by phone, e-mail or fax within 48 hours of receiving the notice to indicate whether they will accept the internship position. Acceptance: After receipt of the call, e-mail or fax, the program director sends each applicant a letter verifying acceptance of the internship position and additional information needed for admission. KU Office of Graduate Studies later sends applicants a letter stating the conditions of admission and admission status.

International Students
Before completing these application materials, please review the Affidavit of Support to make sure you will be able to provide all necessary supporting documentation later on. All international students must complete an Affidavit of Support to establish that they have proof of funding for their studies and living expenses while attending classes at KUMC. The Affidavit and supporting documentation must be provided prior to the issuance of an I-20 or DS2019. Funding requirements are listed on the Affidavit of Support. If you are accepted into a program at KU, an International Student Advisor will contact you via email and request the Affidavit of Support. Note: Do not send this form with your application package.
